How to setup the extension to require subscription confirmation?

Since the extension uses Magento® built-in newsletter to manage the subscriptions, you can required a confirmation email, simply by changing that option the the configuration.

Navigate to "Stores/Configuration" and then "Customers/Newsletter". Change to option "Need to Confirm" to "Yes"
